What this hub is, and what it is not

The public record of health and care reviews is large, and it is public. Statutory inquiries, independent reviews, incident investigations and national strategies are published by government, inquiry websites, NHS England and the devolved administrations. This hub is a Seagry reading of the landmark documents in that record, built so a board, a family or a team can see where the recommendations overlap.

It is not a complete trawl of every document that exists. The NHS also produces Care Quality Commission inspection reports, local patient-safety investigations, coroner prevention-of-future-deaths reports, National Audit Office studies, select committee reports, and the Healthcare Quality Improvement Partnership clinical audit recommendations repository. Those are different collections. Folding them in without a published method would make the hub look complete when it was not. The catalogue is explicit about its boundary: inquiries and investigations that changed, or are changing, national practice, plus the strategies they sit next to.

Text in the hub is paraphrased. It is not the recommendation as published, and it is not a substitute for the report. Each review links to the publisher. Counts of official recommendations are included only where the source states them. Implementation status is a judgement made from public material on 21 October 2026. "Accepted" means a public body said yes. It does not mean the ward has changed.

Dr Penny Dash's review of the patient safety landscape, published on 7 July 2025, said the number of recommendations had itself become a problem, and asked the National Quality Board to keep a repository, a clearing house and a check on impact. The government accepted that recommendation. This hub is an open prototype of the public half of that job. It is not an official DHSC, NHS England or inquiry product.

The catalogue lives in the repository as structured records. On startup the app writes them into SQLite if the seed version has changed. The schema is ordinary SQL, so the same records can move to Cloudflare D1 later without a new data model. Adding a review means editing the catalogue and bumping the seed version.

Seagry partners with NHS, public and third sector organisations on finance, digital and transformation. The hub is a way of putting that work in the context of what inquiries have already asked for. Questions about the method can go to seagryconsultancy.com.
