Infected Blood Inquiry report
Sir Brian Langstaff · work from 2017 · published 2024
What this was
UK statutory inquiry into how people were infected with HIV and hepatitis by NHS blood and blood products, and into the response over subsequent decades. It found a systemic failure to put patient safety first, and a response that added to the harm.
Why it sits in the hub
It is the largest treatment-disaster inquiry in the UK. Compensation, candour and the duty of the state to tell the truth are now being tested in implementation, not only in the report.
Findings
Finding 1
Patients were infected when risks were known, and were not told.
Finding 2
The decades after the infections compounded the harm through delay, denial and inadequate support.

Pay compensation through a scheme that is independent and prompt.
The Infected Blood Compensation Authority was established. Payment has been phased.

Put a duty of candour on the state and its agencies, not only on healthcare providers.

Remember the victims, and change the culture that preferred institutional reputation.
